We are a small nursing service providing advice, education and support for adults with Chronic Obstructive Pulmonary Disease (COPD).
We are a non-acute nurse led service providing advice, education and support for patients with COPD.
We have a clinic in Radcliffe Primary Care Centre one day a week . We predominantly see patients in their own home.
After initial referral we contact the patient for a telephone assessment, then decide if a home visit is required.
The ethos of our service is self-management, giving individuals the knowledge and help them develop skills and coping strategies to manage this chronic disease.
